Template wording for patient information sheets

What is it?

It is critical that potential research participants get clear and balanced information about what would happen if they stop taking part in a study, or if they lose contact with the researchers during a study.

We have worked with patients to produce template wording that can be adapted for different studies.

This wording meets all the PeRSEVERE recommendations. It is also consciously ‘layered’, so that all patients receive the same short, key points, with more information readily available for those who want it.

Anyone involved in creating or reviewing information intended for potential research participants.

The template wording is also available in several other languages commonly spoken in the UK. We provide these just as an example of good, inclusive practice. But they should be treated as a starting point – while the English text was translated by professional translators who were asked to keep the wording accessible, we strongly encourage anyone using the wording to work with public contributors to ensure the wording is right for their study.
